As part of a broader drive to reduce pressure on NHS services, we're offering a supervised, high-quality exercise programme designed specifically for individuals living with – or at risk of – long-term health conditions.

This fully tailored programme begins with a comprehensive health check. Qualified coaches then create and deliver personalised exercise plans based on each individual’s medical history, physical ability, and personal preferences. These exercise activities may include: gym - based exercise, group exercise classes, swimming or sports sessions.

Referral inclusions include:

Low Risk Medium Risk High Risk
  • Type 2 diabetes (diet and/or pharmaceutically controlled).
  • Mild Osteoarthritis / Rheumatoid Arthritis.
  • Depression, anxiety and stress (mild/moderate).
  • Mild asthma.
  • Overweight (BMI > 25) with no other risk factors.
  • Skeletal and muscular injuries (mild/moderate).
  • Hypercholesterolemia.
  • Hyper/hypothyroidism.
  • Seropositive HIV.
  • Perinatal (no other risk factors).
  • Stage one hypertension 140/90-159/99 (medication controlled).
  • Osteoporosis.
  • Physical disabilities (no other risk factors).
  • Neurological problems including Stroke, Parkinson’s and Multiple Sclerosis (stable).
  • Depression/anxiety (moderate).
  • Controlled diabetes type 1 or type 2.
  • COPD (without ventilatory limitation).
  • Chronic fatigue syndrome.
  • Fibromyalgia.
  • Moderate Osteoarthiritis or Rheumatoid arthiritis.
  • Early symptomatic HIV.
  • Recent stroke (within 3 months).
  • Individuals at high risk of falls.
  • Claudication.
  • COPD/Emphysema (with ventilatory limitation).
  • Severe Osteoarthirits or Rheumatoid arthritis.
  • Advanced diabetes (type 1 or 2).
  • Psychiatric illness/cognitive impairment/dementia.
  • Cancer.
  • Recent heart attack (within 3 months).
  • Heart disease or heart condition.
  • Hypertension: Systolic >180 and Diastolic >110.

Participants can access the programme free or at a reduced cost, dependant on individual circumstances. Those in receipt of Universal Credit, Pension Credit or Disability allowance can access the service free for 6 months.

Sessions are available at Surrey Sports Park, the University of Surrey campus, and Community venues across Guildford.

Ask your GP, Social Prescriber, Children’s & Adult Services, School Nurse, Midwife, other Healthcare Professional, and various Community Partners today for a referral.
